Questionnaire

The questionnaire is the first step for any new case with Envoy. Find more details on how the questionnaire works in your Envoy account.

How do I complete the questionnaire? 

Every case prepared by CIP in the Envoy platform begins with a questionnaire for you and/or your employer to complete. The questionnaire must be 100% complete by all parties and submitted for review before the legal team will begin working on a case.

We ask that you complete the questionnaire to the best of your ability, and upload as many documents as you have which apply to the case. The questionnaire is used internally to gather information that the legal team needs to assess the case, update the list of documents needed, and draft the Case Materials. If the legal team requires any additional information or documentation from you, they will let you know after their initial review.

If you need help while working on the questionnaire, click on the ‘i‘ for information icons. These will show you help text with additional information to aid you with the question you are answering.

What if a question doesn’t apply to me? 

If there is a question that does not apply to you, or you do not have the appropriate information yet, please enter ‘N/A’ or ‘Not Applicable’ – this will satisfy the request so you can submit the case for review. If a field contains text restrictions, such as a date or numerical value, please enter null information.

The questionnaire must be 100% complete by all parties before the case can be submitted for review. After the legal team completes their initial review, they will let you know if anything else is needed. If the legal team requires additional explanation on the information you have submitted, you will be notified.

 

What if I don’t know what a question is asking of me? 

 

If you have any questions on a specific section, question, or document on your questionnaire, please look to the ‘i‘ for the information icon.

Please complete the Questionnaire to the best of your ability and upload as many documents as you have which apply to the case. The questionnaire is used internally to gather information that the legal team needs to begin preparing the case. By providing as many applicable documents as possible, the legal team will also cross-reference the information in the questionnaire with the documentation provided to collect the most accurate and current information.

I’ve already completed the questionnaire for a previous case. Do I need to complete a new questionnaire?  

Any time a new case is opened, there is a new questionnaire. It is pertinent that you complete each questionnaire in its entirety. If you have completed a questionnaire previously, some information and documents will populate into the new case questionnaire.

We will always want to ensure that the most accurate and up-to-date information is provided with each case questionnaire, so you should always plan to review and complete the new questionnaire any time that a new case is opened in your account.

If you log into your account and find that the case is already under review, this means that the legal team or your employer pushed the case forward, and no additional information will be required from you at that time.

 

When can I expect an update on the case once it’s been submitted for review? 

When you or your employer submit the questionnaire, you will be alerted that the case has been moved to the legal team for review. You can anticipate similar notifications as the case progresses. Your account will send you an update when your case status changes.

The legal team will complete an initial review of the documents and information that you and your employer have provided. You will be notified of the next steps after the legal team has completed this review process if anything further is required from you.
